學生宿舍申請表 (只限本科生)
Application Form for Student Dormitory (for Undergraduate Students only)

申請於填寫網上申請前，需先行準備以下文件：
Applicants should prepare following documents before filling the online form:

  • 証件相(<600KB，檔案類型：PNG, JPG)
    A recent ID photo (<600KB, FORMAT: PNG, JPG);
  • 三個月內的住址証明 *(<2MB，檔案類型：PNG, JPG)
    *現居於學校宿舍的非本地生，請提供以下其中一項文件作住址証明：
    1. 宿生証副本
    2. 2019-20下學期宿費收據副本
    3. 2019-20下學期宿費付款通知書副本
    Official address proof issued in last 3 months *(<2MB, FORMAT: PNG, JPG);
    *Non- local students who currently live in student dormitory please provide one of the following supporting documents:
    1. Copy of resident card
    2. Copy of 2019-20 term 2 residence fee receipt
    3. Copy of 2019-20 term 2 residence fee debit note
  • 出生證明(如適用) (<2MB，檔案類型：PNG, JPG)
    Birth certificate (if applicable) (<2MB, FORMAT: PNG, JPG);
  • 成績証明 (<2MB，檔案類型：PNG, JPG)
    Academic records certificate (<2MB, FORMAT: PNG, JPG);
  • 職務證明文件或「非學術成就」証明書(如適用) (<2MB，檔案類型：PNG, JPG)
    Non-academic achievements certificate (if applicable) (<2MB, FORMAT: PNG, JPG);
  • 由醫生發出的健康證明文件(如適用) (<2MB，檔案類型：PNG, JPG)
    Any supporting documents and recommendation from a medical practitioner (if applicable) (<2MB, FORMAT: PNG, JPG);

因系統需要時間處理申請資料，請申請人預留足夠時間提交申請，以免造成延誤。
To prevent delay submission, applicants should reserve enough time for the system processing the application.

  1. 本處不會就任何因操作系統及互聯網瀏覽器組合所引致的系統故障而引致宿位申請出現延誤或錯誤承擔任何責任。
    SAO will not be liable for any errors in submission of application due to the malfunction of operation system / internet web browser combinations.
  2. 本人已仔細閱讀及明白「學生宿舍2020-21學年申請須知」及「學生宿舍2020-21宿生守則」。
    I have read thoroughly and understood the “Student Residence 2019-20 Application Guide” and “Student Residence 2019-20 Rules and Regulations”.
  3. 本人謹此聲明已瞭解所提供個人資料之用途，並授權珠海學院使用本回執內有關的個人資料，作處理申請學生宿舍之用。
    I declare that I understand the use of my information submitted and I authorize Chu Hai College of Higher Education to use my personal information for processing my application for student residence.
  4. 本人謹此聲明，以上所填報的資料均屬真確無誤及完整，本人明白如有任何虛報或誤導資料，學院有權撤銷本人的申請。
    I declare that all information submitted above is complete and accurate. I understand that the provision of any false or misleading information will lead to disqualification of my application.
